Admission

Step-by-Step Admission Process

1. OPD Consultation & Doctor’s Advice

  • Admission is initiated after consultation with the treating doctor.

  • If inpatient treatment is required, the doctor provides a written admission advice (recommendation slip).

2. Admission Counter Registration

  • Visit the Admission Counter along with the doctor's advice slip.

  • Submit valid ESI Card / Pehchan Card / Aadhaar for verification.

  • Basic details are recorded, and a Bed Admission Form is issued.

3. Bed Allocation

  • Based on availability and clinical condition, a bed is assigned in the appropriate ward (General / ICU / HDU / Special ward).

  • Emergency and critical cases are prioritized.

4. Inpatient File 

  • A unique IP (Inpatient) file is created with patient history and treatment orders.

5. Admission to Ward

  • The patient is taken to the designated ward with the necessary file and personal belongings.

  • A nurse and ward in-charge assist in settling the patient and initiating treatment.

You are being directed to CPGRAM website. You would be asked to Register with CPGRAM if not already registered. Registered users can login directly using their CPGRAM user ID and password.

  • 1. Login into CPGRAM
  • 2. In section "Grievance Detail" select "Others/Not Listed/Not Known"
  • 3. In the "Ministry/Department" Drop Down Select ESIC
  • 4. In "Subordinate Department/Office" select the Regional Office
  • 5. Provide details of the complaint
