Step-by-Step Discharge Process
1. Discharge Recommendation by Treating Doctor

The treating doctor assesses the patient’s recovery and recommends discharge when the condition is stable.

A final round of medical evaluation is done before initiating the discharge process.
2. Preparation of Discharge Summary

A discharge summary is prepared by the attending doctor or medical team.

It includes the diagnosis, treatment given, medications prescribed, lab reports, and follow-up instructions.
3. Clearance from Departments

Before discharge, clearance is obtained from:

Ward In-Charge (to ensure all hospital belongings are returned)

Pharmacy (if medicines are to be collected)

4. Patient/Attendant Briefing

The patient and/or attendant is briefed about:

Medication instructions

Diet guidelines

Follow-up schedule

Emergency symptoms to watch for

5. Discharge Approval

The final approval for discharge is given by the Medical Officer/Ward In-Charge.

The discharge summary and any original documents are handed over to the patient.
6. Exit from Ward

After all formalities, the patient exits the ward with support (if required) and leaves the hospital.
