Responsibilities of Patients

  1. Provide Accurate Information

    • Share complete and truthful medical history, current symptoms, and past treatment records.

  2. Follow Treatment Instructions

    • Cooperate with the treating doctor and staff by following prescribed treatment, medication, and dietary guidelines.

  3. Respect Hospital Rules & Staff

    • Maintain decorum, follow hospital rules, and treat all staff with respect.

  4. Avoid Self-Medication

    • Do not take or insist on unprescribed drugs or procedures.

  5. Report Changes

    • Inform the doctor or nurse about any sudden changes in condition.

  6. Maintain Hygiene

    • Follow hygiene protocols and use hospital facilities responsibly.

  7. Use Resources Responsibly

    • Avoid misuse or damage of hospital property, beds, wheelchairs, and medical equipment.

  8. Timely Reporting for Appointments

    • Arrive on time for check-ups, investigations, or admissions.

 Responsibilities of Attendants

  1. Follow Visiting Hours

    • Comply with scheduled visiting hours and instructions from security or ward staff.

  2. Limit Crowd

    • Only one attendant is allowed with the patient; avoid overcrowding.

  3. Cooperate with Medical Staff

    • Follow directions given by doctors, nurses, and administrative staff.

  4. Avoid Interference

    • Do not interfere with clinical procedures or try to influence treatment decisions.

  5. Respect Other Patients’ Privacy

    • Maintain silence and avoid using mobile phones in wards or treatment areas.

  6. Do Not Offer Tips or Gifts

    • Refrain from offering money, gifts, or favors to hospital staff.

  7. Maintain Cleanliness

    • Help keep the hospital clean by disposing of waste in designated bins.

  8. Report Problems or Concerns

    • Use the Help Desk or Grievance Cell to report any issue in a respectful manner.

Instructions

You are being directed to CPGRAM website. You would be asked to Register with CPGRAM if not already registered. Registered users can login directly using their CPGRAM user ID and password.

  • 1. Login into CPGRAM
  • 2. In section "Grievance Detail" select "Others/Not Listed/Not Known"
  • 3. In the "Ministry/Department" Drop Down Select ESIC
  • 4. In "Subordinate Department/Office" select the Regional Office
  • 5. Provide details of the complaint

Proceed
